2- spleen examination

بـ tip of your fingers تبدي من Rt iliac
لحـد Lt hypochondria وتقول للـ pt كل مره ياخد Deep breath
لما توصل للـ  Lt hypochondrium  ادير  
shortness maneuver

تقول للـ pt يتلفت  ع Lat side
ادخل يدكـ تحت Lt costal margin
لو يدكـ خشت معناها الـ pt معنداش
Splenomegaly
ولو مخشتش معناها في splenomegaly💔

وهنا ادير percussion من
Lt hypochondrium وتنزل لـ Rt iliac
فتلقى الصوت قعد dull بدل ميكون hyper_resonance نتيجة الـ stomach gas
ووجود dullness يثبتلي وجود
splenomegaly

الـ percussion مديراش الا لو لقيت splenomegaly

3- kidney

اـو لقيت مفيش splenomegaly
وشكيت ان الـ enlargement in kidney
فهنا انديروا الـ Ballotting test

Percussion

من تحت  xiphoid process ننزل بالـ percussion لحد الـ umbilical
كله الصوت المفروض والطبيعي ان يكون resonance بعدين من الـ umbilical ناخد لجهة Rt or Lt ونكمل percussion لحد
Ant . axillary line
هدا في حال معنديش اي organomegaly

اما لو كان في organomegaly علي وحده من الجهات فالـ percussion متعي بكون في الـ side التاني الي مفيشي الـ enlargement ،،

فرضاً لما مشيت للـ Lt side لقيت الـصوت قعد dull فـ ممكن واحد من الأسباب ان يكون fluid or mass

fluid فلو كان
نقول للـ pt يقلب ع الجنب التاني
ونراجي 1min عشان لو كان fluid
رح ينزل بالـ gravity للجانب التاني ولما نعاود ندير percussion نلقى الصوت رد
Resonance
وهدا نسموه shifting dullness test

ولازم ندير نفس الكلام علي other side عشان تشوفه هل هو unilateral or bilateral
لأن في ascites تتجمع في جيه وحده زي في حالات :
hydatid cyst
Ovarian cyst
TB ascites

في حال طلع الـ +shifting dullness is ve
اندير Fluid thrill test
ولو كان حتى هو +ve فهو دليل علي
Sever or moderate ascites

Examination of Hernia :

أول شي ال pt يكون واقف واندير exposure
من umbalicus to mid thigh

By inspection :

¹ site = have swelling in Rt groin area

² size = large in size

³ shape = globular or pyriform  in shape

⁴ skin over = No Redness,  no scar

⁵ نقوله معليش كح !!

Superficial palpation :

No hotness,  no Redness

Deep palpation :

¹ there's have swelling in Rt groin area

² large in size

³ globular in shape

⁴ smooth surface

Soft and elastic when content is intestine
Doughy when content is omentum
Tense and tender obstructed hernia

⁵ firm in consistency

⁶ not attached to skin or underlying structure

⁷ نقوله معليش كح !!

By percussion = Resonant or dull

Resonant = enterocele
Dull = omentocele

By Auscultation = bowel sound

Bowel sound over the swelling suggests enterocele

Special test :

¹ Reducible test
تقدر تردها مكانها ؟ كيف ترا ؟

² deep occlusion test
تحط يدك ع مكانها وتقوله كح

³ zeman test
كل الأماكن المتوقعه للهيرنيا نسكرهم ونقوله كح

1- Introduce yourself to the patient
2- take permission for examination
3- Respect the privacy of the patient

تحترم خصوصية المريض في إنك ما تكشفش عليه قدام حد وتكون في حجرة مسكره

بالطريقة اهيا :

السلام عليكم
كيف حالك يا عمي أني طالبة سنة خامسة  من بعد إذنك نبي نكشف عليك لو تسمحلي 

1- لازم المريض يكون واقف ( in standing position )

2- لازم يكون لابس كندره أو شبشب لأن مرات الوطا تكون مصقعه أو الجو صقع فـ الـ cremasteric ms
يصيرلها contraction ومعاش تقدر تكشف عليها

3-  Full genetalia should be exposed

By inspection :

¹ have a penis circumcised

² meatal opening on top of penis

³ meatal orifices on central
No meatal stenosis or discharge

⁴ no skin changes ( vesicle or ulcer )

⁵ Scrotal well developed or not

⁶ raised or descended

⁷ any skin changes ( redness or scar )

⁸ swelling ( site , size , shape , skin over )

⁹ نرفع scrotum  ونشوف tb sinus

By palpation

نجي قدام المريض ونثني رجليا وننزل بحيت مستوي عيوني يكون ع genitalia

Superficial palpation :

no Redness, no hotness

Deep palpation  :

نفس اي swelling
( site , size , shape, surface ,  consistency )

If cystic consistency do 2 test

Fluctuation test
Transllumination test

Palpate spermatic cord,  epididymis,  testis

Then valsalva

Note :
ماتنساش ادير نفس الخطوات ع الجهة الثانية وديما نبدو بالنورمال هي الاولي
