🛑Overall, hypokalemia is more frequent with thiazide diuretics than with loop diuretics, despite having a less potent natriuretic effect
✅ One potential explanation is the differential effect on calcium excretion. Whereas thiazide diuretics decrease calcium excretion  loop diuretics lead to a significant increase in calcium excretion The higher luminal calcium concentration in the distal nephron produced by loop diuretics reduces the lumen-negative driving force for potassium excretion , thereby blunting their kaliuretic effect of loop diuretics.

🛑Management of LDL-C to reduce cardiovascular risk in nondialysis CKD patients


🛑Statin for secondary prevention
if A patient has established atherosclerotic CVD (prior history of coronary, cerebrovascular, or peripheral arterial disease)


🛑Statin for primary prevention 


⛔️Option A

✅All patients with GFR <60 mL/min

✅Patients with CKD and GFR ≥60 mL/min and age ≥50 years

or

✅Patients with CKD and GFR ≥60mL/min and other CVD risk factors (eg, diabetes, hypertension, smoking, low HDL-C, high Lp(a), etc

⛔️Option B*:

✅Statin if predicted 10-year absolute risk of having a major CVD event is 7.5 to 10% or higher

✅Statin might be offered if risk is between 5 and 7.5%

✅No statin if risk is below 5%

في حاله ان المريض مركب

لكن عنده
significant ischaemic stroke and are at risk of haemorrhagic transformation
تقزل ال NICE البريطانيه انه نعمل
Hold for anticoagulation for 1 week and start aspirin 300mg
اما اذا كان عنده
non-valvular AF or paroxysmal AF
نوقف ال DOAC لمدة 14 يوم

بينما المريض الي عنده
VTE (DVT, PE)
هنا نحدد غ حسب المريض هل هو حاد او لى هل نستخدم

نحن ندرك أهمية دواء betamethasone او dexamethasone للجنين الذي بيكون معرض للولادة المبكره patients at risk for preterm birth عشان نقلل severity of respiratory distress syndrome (RDS)
لكن اذا كانت الام الحامل مصابة بارتفاع ضغط الدم الدم لأي سبب تسمم حملي او غيره هل تعتبر هذه الادوية ممنوعه ؟

الاجابه لا
✅ اولا دواء betamethasone و dexamethasone اقل mineralocorticoid activity مقارنة ببقيه ادوية corticosteroids لذا تاثير على ضغط الدم اقل

🛑For patients at intermediate risk for ASCVD (ie, 10 to 20 percent risk at 10 years) who do not otherwise qualify for lipid-lowering therapy, we suggest a screening measurement of hs-CRP. We take the following approach to the interpretation of these values:
✅A value above 10 mg/L should prompt consideration of a source of infection or inflammation, with repeat measurement of hs-CRP in two weeks.
✅A value of 3 to 10 mg/L should prompt a discussion regarding overall ASCVD risk, lifestyle modifications, management of risk factors for atherosclerosis, and the possibility of risk reduction therapy using a statin.
✅A value of 1 to 3 mg/L should be repeated in two weeks to determine an average value and prompt a discussion regarding lifestyle modifications and the potential for treatment with a statin if the hs-CRP value is 2.0 mg/L or greater.
✅A value <1 mg/L suggests a lower risk of future ASCVD events.

وجد أن الأشخاص ذوي الدخل الاقل كانوا اكثر عرضة لأمراض الضغط والسكر واستخدام السجاره مقارنة بالأشخاص ذوي الدخل الأكثر
بينما ذوي الدخل العالي كانوا اكثر عرضه لسكر والسمنه المفرطة

🛑كيف يتم معالجة نقص فيتامين دال ؟

✅حسب تركيز serum 25(OH)D

🛑اولا  لو كان serum 25(OH)D اقل من 12ng/ml
او مايسمى ب Vitamin D deficiency)

هنا نعطي في الغالب من 25,000 إلى  50,000 وحده من فيتامين D2 او D3 (cholecalciferol)  أسبوعيا لمده سته إلى ثمانيه اسابيع ثم  نعطي 800 وحده يوميا ،نقيم المعالجة بعد ثلاثه إلى أربعة أشهر ونسوي فحص لتركيز serum 25(OH)D


🛑ثانيا لو كان تركيز  serum 25(OH)D مابين 12 إلى اقل من 20 ng/mL (او مايسمى ب Vitamin D insufficiency)

هنا نعطي من 800 إلى 1000 وحده يوميا
ممكن نقيم المعالجة بعد 3 اشهر

🛑about baclofen drugs

✅Gradual dose reduction is advised unless discontinuation is due to significant adverse effects or overdose-related emergencies;
✅chronic oral baclofen therapy should be discontinued slowly (eg, over at least 1 to 2 weeks or longer if withdrawal symptoms occur)
❌ Abrupt discontinuation of oral or intrathecal baclofen may result in severe adverse effects, including multiple organ system failure and death

🛑استخدامات دواء colchicine في مرضى القلب كالاتي
✅لدواء colchicine استخدامات عديده في مرضى القلب نذكرها باختصار

✅اولا في حاله التهاب التأمور pericardial disease
✅في حاله acute and recurrent pericarditis يوصى الجايدلاين الاوروبي ESC باستخدام دواء colchicine خيار اول بينما الجايدلاين الأمريكي يوصى باستخدام دواء colchicine في حاله كان pericarditis بعد MI
✅الجرعه في هذه الحالتين حسب الوزن لو كان وزن المريض اقل من 70 كيلو ياخذ 0.5mg مره في اليوم لو وزنه اكثر من 70 ياخذ نفس الجرعه ولكن مرتين في اليوم لمده ثلاثه أشهر
✅ممكن في حاله
🛑Post-Pericardiotomy Syndrome pericarditis
🛑Dressler's syndrome pericarditis
لمده شهر بنفس الجرعه المذكوره

🛑ثانيا في حاله ACS او chronic coronary syndrome
✅متى بضبط؟
ذكرته في منشور سابق ؛لمنع Secondary attack في حاله ضبطنا جميع risk factors ل ACs مثل dyslipidemia و hypertension و DM ..الخ
هنا ممكن نعطي 0.5mg في اليوم قد نستمر به مدى الحياة

🛑اخيرا هناك استخدام غير مشهور ولكن قد نستخدمه لمنع atrial fibrillation بعد العمليات الجراحية او Post-ablation atrial fibrillation بجرعه 0.5mg مره إلى مرتين في اليوم لمده شهر إلى شهرين

بالمختصر كان في مرضى ACs يكونوا مضبطين كل risk factors
مثل diabetes و Hypertension و atherosclerosis
عشان مايدخلوا نوبه ثانيه من ischmic
وملتزمين بالعلاج و optimal dose

ومع هذا كانوا يدخلوا في نوبات ثانيه🙁😁

فهم بحثوا  عن اسباب

ولقوا مثلا ان inflammation mediator مرتفعه
وان  CRP كان مرتفع قوي

في هذه الفئه من المرضى بيكون colchicine مفيد👏
