Deformity of Foot

¹ congenital talipes equino varus
= club foot

² hammer toe
³ claw toe
⁴ mallet toe

⁵ pes cavus = high arched foot
⁶ pes planus = flat foot

⁷ hallux valgus = deviations of big toe away from midline at metatarsophalengeal joint

Club foot = Congenital talipes equinovarus

هو اعوجاج في كاحل القدم خلقي في العادة يصيب الاولاد  ، وهو اكتر حالة تصيب الجهاز العظمي في المواليد و بنسبة 1:1000 , الاعوجاج يكون للداخل زي ماهو واضح في الصورة .
الأسباب غير معروفة إلى الآن ،  مافيش مشكلة جينية واضحة ولو انه 25% من الآباء يلي كان عندهم الاعوجاج صار لأولادهم  ، كان فيه نظرية أنه قلة ماء الرحم خلال الحمل ممكن يكون السبب لكن لم يثبت ذلك  .
المشكلة سببها الأساسي قصر في انسجة العضلات و الاربطة تسبب بذالك اعوجاج في العظم .
من الممكن اكتشاف المشكلة خلال فترة الحمل
العلاج 90% من الحالات غير جراحي بس جبس يبدأ من نهاية عمر الاسبوع التاني لفترة حوالي شهر و نص إلى شهرين  بعدها دعامات معينة لغاية عمر 4 سنوات ليعود بعدها الطفل بشكل طبيعي جدا

Hallux valgus = deviations of big toe away from midline at metatarsopharengal joint

Common in women 💅

Cause = congenital or narrow pointed shoes

Ttt = wide shoes and insert wedge of plastic b/w 1st , 2ed toe.

D/D of painful hip:
1 septic arthritis.. In neonate
2 perthe's disease.. In children
3 transient synovitis.. In children
4 slipped upper femoral epiphysis
5 osteoarthritis >30 y
6 fracture of neck of femur... In elderly

Perthe's disease :

Unknow cause But sometimes is due to mild effusion which lead to AVN in head of femur➡️deferomity in femoral head become flat & Short neck
⛔Common in children more in boys (MCQ)
⛔ Investigation- lab is normal
-Xray in early stage is normal( MCQ)
Late stage is flat irregular head & Short femoral neck
-radioisotop scanning  (cold spot) best Diagnostic
⛔Treatment
If child <6y only supportive
If 6.-8 y supportive +abduction brace(Broom stick brace) MCQ
If >8 surgical intervention.

Causes AVN of femoral head :

1. Joint or bone fracture
Fracture neck of femur or dislocation
2. SUFT
3 perthe's disease
4. TTT of DDH (sever abduction)
5. Steroid use ( is common causes of AVN)
6.Gaucher's disease
7. Sickle cell Anemia

🚨Fracture neck of femur:

⛔If intracapsular fracture main complain is AVN.
⛔If extracapsular  main complain is malunion.
⛔Fracture of neck of femur
Leg is abduction & external rotation.

💡Intracapsular fracture is classified by #Garden's classifiction:

⭕Grade 1 incomplete fracture.
⭕Grade 2 complete fracture without displacement.
⭕Grade 3 complete fracture with partial displacement.
⭕Grade 4 complete fracure with complete displacement.

⛔Grades 1.2 are low risk AVN.,ttt is by 3 cannulated scerw
⛔Grades 3.4 are high risk to AVN
ttt is If pt >65y ➡️ hemiarthroplasty.
If <65y or osteoarthritis of hip➡️ Totol hip replacement.

⛔️Complication of fracure of Femur shaft:

-Vein&nerve Injury.
-Fat embolism.
-Delay nonunion or malunion.

⛔Supracondylar fracture of femur:

Due to high enegry trauma.
-Ttt by open reduction internal fixation.
May occur Injury to femoral A & popliteal A.
