✅ A serum-ascites albumin gradient (SAAG) is a useful test for differential diagnosis of ascites.  Both should be drawn/measured at same time
🛑. High gradients (≥ 1.1 g/dL) is seen in patients with:
✅Cirrhosis
✅Cardiac ascites
✅Mixed ascites
✅Alcoholic hepatitis ✅Budd-Chiari Syndrome
✅Fulminant liver failure
✅ Fatty liver in pregnancy
✅Veno-occlusive disease
✅Portal vein thrombosis

🛑 Low gradient < 1.1 g/dL is seen in:
✅ Tuberculosis peritonitis
✅Pancreatic ascites
✅Biliary ascites
✅ Nephrotic syndrome
✅ Peritoneal carcinomatosis

🛑At what level of WBC, Platelet and Hematocrit count is pseudo hyperkalemia expected?
✅Pseudo hyperkalemia is usually a laboratory artifact rather than a real biological it can also occur in patients with abnormally high numbers of the following

✅Platelets count (>500,000/mm³)
✅ Patient with leukocytes (> 70 000/mm³)
✅Patient with increase erythrocytes (hematocrit > 55%)

✅Patients with severe vitamin D deficiency  (<12ng/ml  )are at risk for developing osteomalacia
🛑 In such patients, we measure:
✅Serum calcium
✅, phosphorus, alkaline phosphatase
✅parathyroid hormone (PTH)
✅Electrolytes, blood urea nitrogen (BUN), creatinine
✅Tissue transglutaminase antibodies (to assess for celiac disease)

نصائح للحد من ارتفاع الكوليسترول الضار:

💥يمكن أن يساعد إجراء تغييرات في نمط الحياة على الحد من ارتفاع الكوليسترول الضار في الدم، ويذكر منها ما يأتي:

📍الحد من استهلاك الدهون الضارة:

يساعد تقليل تناول الدهون المشبعة والمتحولة، مثل: اللحوم، ومنتجات الألبان، وزيت النخيل، وزيت جوز الهند، والحليب كامل الدسم، والأطعمة المقلية، واستبدالها بالدهون الصحية أثناء الطبخ مثل
: استخدام زيت الزيتون أو زيت الأفوكادو بدلًا من الزبدة والكريمة على تقليل ارتفاع الكوليسترول الضار في الدم.

📍تناول الدهون الصحية غير المشبعة باعتدال:
مثل: المكسرات، والبذور، والأسماك الزيتية، والأفوكادو، والزيوت النباتية كزيت الزيتون، وزيت الفول السوداني، وزيت الكانولا، وزيت فول الصويا.

📍تناول كميات أعلى من الألياف القابلة للذوبان:
يمكن أن يقلل هذا النوع من الألياف من امتصاص الكوليسترول الضار في الدم، ويبطئ عملية الهضم، ومن الأطعمة الغنية بها:
الشوفان.
البقوليات، مثل العدس، والبازلاء، والفاصولياء.
الشعير.
الفواكه والخضروات، مثل التفاح، والجزر.

📍زيادة مستوى النشاط البدني:
يوصى الأشخاص الذين تتراوح أعمارهم بين 18 و64 عامًا بممارسة نشاط بدني متوسط الشدة لمدة تتراوح بين 30 - 60 دقيقة معظم أيام الأسبوع، بينما يوصى الأشخاص الذين تبلغ أعمارهم 65 عامًا فما فوق بممارسة ما مجموعه 30 دقيقة من النشاط البدني المعتدل معظم أيام الأسبوع.

📍التخلص من الوزن الزائد:
يمكن أن يساعد ذلك على تقليل مستويات الكوليسترول الضار والدهون الثلاثية في الجسم، إلى جانب زيادة مستويات الكوليسترول الجيد.

📍التوقف عن التدخين:
يقلل التدخين من نسبة الكولسترول الجيد مقابل الكوليسترول الضار في الدم، ويساهم في تشكيل الرواسب الدهنية في جدران الشرايين ويزيد من خطر تجلط الدم والإصابة بالنوبات القلبية والسكتات الدماغية

✅1) If AKI develop suddenly in ICU patient without clear history of fluid/blood loss

chloroquine-sensitive
use either chloroquine or hydroxychloroquine.

- Chloroquine phosphate dosing consists of 600 mg base (1,000 mg salt) orally immediately, then 300 mg base (500 mg salt) orally at 6, 24, and 48 hours.

- Hydroxychloroquine dosing consists of 620 mg base (800 mg salt) orally immediately, then 310 mg base (400 mg salt) orally at 6, 24, and 48 hours.

اما اذا كان مش متوفره او عنده
chloroquine-resistant P. falciparum infection.
نستخدم
  use 1 of the following (listed in order of preference):

1. Artemether/lumefantrine 80 mg/480 mg (4 tablets, 20 mg/120 mg each) orally twice daily for 3 days:
ياخذ في البدايه جرعه وبعد 8 ساعات جرعه بعدين جرعه مرتين في اليوم لليومين الأخيره
•لا يستخدم في المرضع او ما كان وزنه أقل من
  infants < 5 kg.
يفضل أخذه مع الاكل لتحسين الامتصاص

2. Atovaquone/proguanil 1,000 mg/400 mg (4 adult tablets, 250 mg/100 mg each) orally once daily for 3 days:

لا يستخدم في الحوامل او المرضعات او  infants < 5 kg.
الا اذا كان مافي خيار اخر او كان  ال
  benefits outweigh the risks.
يفضل استخدامه مع الاكل لتحسين الامتصاص

3. Quinine sulfate:
• Quinine sulfate may be available as 542 mg base (650 mg salt) or, in the United States as capsules containing 324 mg salt (therefore 2 capsules are equivalent to 1 oral dose).
⚫ The frequency and duration of quinine sulfate is 3 times daily for 3 days (or 7 days if infection acquired in Southeast Asia) plus 1 of the following:
о Doxycycline 100 mg orally twice daily for 7 days
• Tetracycline 250 mg orally 4 times daily for 7 days
Clindamycin 20 mg/kg/day orally divided into 3 doses daily for 7 days


4. Mefloquine 684 mg base (750 mg salt) orally as initial dose, followed by 456 mg base (500 mg salt) orally 6-12 hours after initial dose:
This option is not recommended for patients acquiring infection in Southeast Asia due to resistance.
• Only consider this option if other treatment options are not
available.
Mefloquine is not recommended for patients with
neuropsychiatric history.

✅لذا مريض العناية الذي لديه Lower limb edema مش يعني انه عنده نقص البومين ،يمكن بسبب زياده الصوديوم الناتج عن maintenance fluid 😅 لذا من المهم انه ياخذ sodium balance بعين الاعتبار نفس fluid balance
