🛑treatment insomnia in palliative care
✅If a patient has insomnia due to delirium (eg, accompanied by restlessness, agitation, or psychosis), prescribing zolpidem or benzodiazepines or using the over-the-counter agent diphenhydramine to treat insomnia may worsen the delirium.
✅Patients with end-stage chronic obstructive pulmonary disease (COPD) may experience worsened respiratory function when prescribed benzodiazepines.

✅. For patients with insomnia related to pruritus, doxepin may help treat the pruritus as well
✅Mirtazapine  mirtazapine is associated with sedative side effects (at least at doses of 7.5 to 15 mg), which may be useful for the treatment of the depressed or anxious patient also experiencing insomnia

🛑بنسبه ل كيفيه إعطاء   جرعه حقن ادوية  aminoglycosides 

✅اما طريقه traditional intermittent dosing بحيث تعطي بجرعه صغيره مرتين الى ثلاث مرات في اليوم(هذا الشي في المريض الذي لديه وضائف الكلى في المستوى الطبيعي )

✅او بطريقه an extended interval dosing
بحيث تعطي بجرعه عاليه مره في اليوم الى مره كل ثاني يوم (حسب وظائف الكلى )،


🛑هل نوع العدوى البكتريا تحدد كيفيه إعطاء جرعه حقن ادوية aminoglycosides؟

✅الاجابه نعم ونفصلها كالاتي قي مريض الذي لديه وضائف الكلى في المستوى الطبيعي

✅اولا اذا كانت البكتريا سالبة الجرام
For gram-negative bacterial infection

هنا نفضل إعطاء هذه الادوية بطريقه extended interval
we suggest extended-interval dosing rather than traditional intermittent dosing. 

✅ثانيا في البكتريا موجبة الجرام
For gram-positive bacterial infections

ممكن نستخدم اي طريقه اما extended interval او traditional intermittent يعتمد على pathogen الذي يتم علاقه مثل

✅For invasive enterococcal infections (such as endocarditis), traditional intermittent dosing of gentamicin is preferred when the agent is   used for synergy
نفضل طريقه traditional intermittent بسبب عدم وجود دراسات كافيه ل طريقه extended interval


✅For endocarditis due to Streptococcus species, extended-interval dosing of gentamicin (3 mg/kg/day as a single daily dose) is preferred when the agent is used for synergy

🛑تعالوا نتكلم عن موضوع مهم بخصوص
prolonged infusions of beta-lactams
✅هل ممكن إعطاء المضادات الحيوية من عائله beta lacatam بطريقه prolonged infusions (من ساعتين الى اربع ساعات )افضل من الطريقه التقليدية التي تكون خلال دقائق الى نص ساعه ؟؟

✅من حيث مبدأ pharmacodynamic والتفسير النظري ممكن افضل طيب ليش ؟
✅اولا هذه الادوية تاثير هذه الادوية ل القضاء على البكتريا بيكون time-dependent effect
✅ثانيا من مبدأ انه يهمني ان يكون pathogen يكون exposed ل المضاد طوال المعالجه وهذا بيخلي free drug remains above the minimum inhibitory concentration (MIC) طوال الوقت عشان احقق bacterial eradication وكل هذا بيتحقق من خلال prolonged infusions

🛑طيب من الفئة من المرضى الذين بيتسفيدوا من طريقه prolonged infusions
✅تقريبا بيكون من المرضى الذين risk of drug-resistant pathogens and/or patients with severe infections( غالبا مريض في المستشفى أو العنايه المركزه ) بتوضيح اكثر

✅Gram-negative organisms with higher MIC

✅Structural lung disease (including cystic fibrosis)
✅Frequent healthcare exposures
✅Prior repeated antibiotic exposures
✅Critical illness with severe infection (including central nervous system infections necrotizing fasciitis, neutropenic fever and burns)

🛑ليش مريض العنايه المركزه بذات لعده اسباب تتعلق ب pharmacokinetics مثل

✅Alteration in drug clearance

✅Change on volume of distribution

✅Alteration in protein binding
🛑طيب ماهي عيوب هذه الطريقه

✅اولا مشكلة Stability ل بعض الادويه فمثلا دواء meropenem الذي بالاسم التجاري Merrem يقول بان الدواء اذا حضر ب normal saline بيكون stability فقط لمده ساعه في درجه حراره الغرفه (ونحن ممكن نعطي prolonged infusions ل meropenem خلال ثلاث ساعات ؟؟؟، لكن بعض الدراسات قالت إنه ممكن meropenem ممكن يكون stability بعد التحضير لمده 12 ساعه في درجه حراره الغرفه حتى at concentrations up to 20 mg/mL)

✅ثانيا ماذا من شان الدراسات السريرية بخصوص الموضوع ؟
✅اغلب Clinical data قالت بأنه مافيش فرق في الفعاليه في الطريقتين لكن هناك فرق بأنه prolonged infusions ممكن تقلل من mortality مقارنه بالطريقه التقليدية في مرضى العنايه المركزه
✅على الرغم من اني اخر دراسه اسمها MERCY نشرت في مجلة JAMA الامريكيه بأنه إعطاء continues meropenem لم يحسن من الوفيات قي مريض sepsis مقارنه ب الطريقه التقليدية بل وانه مقاومه المضادات ظهرت في اليوم 28 من استخدام continues meropenem
✅لكن في رأي الخبراء مازلت prolonged infusions هي الأفضل في الفئات المذكوره اعلاه

🛑هناك عبارة تقول التليف الكبدي يعالج ارتفاع ضغط الدم (cirrhosis cures hypertension) ماذا يعني ذلك؟
✅مريض التليف الكبدي بتحديد cirrhosis decompensate  ال systemic vascular resistance and mean arterial pressure (MAP) يحصل لها انخفاض وبصمت خاصه المريض الذي هو obesity و 
cirrhosis due to nonalcoholic steatohepatitis
✅لذا في مريض decompensate cirrhosis يجب ايقاف ادوية الضغط اذا كان MAP اقل من 82 لانه وجدوا في حاله كان MAP في هذا المريض اقل من 82  يكون نسبه two-year survival فقط 20%

✅لذا وجب التنبيه بخصوص مريض ضغط الدم المرافق للتليف الكبدي الحذر في صرف ادوية الضغط لانه غالبا مريض الضغط لايحضى بمتابعة على المدى البعيد

✅ماذا عن دواء propranolol  المستخدم في مريض التليف prevent variceal hemorrhage ؟
✅يجب عدم استخدامها اذا كان مريض التليف
مصاحب ل استسقاء مقاوم ل المدرات (especially diuretic-resistant ascites.)
لانه تخفض الضغط مما يجعل fluid ascites مقاومه ل المدرات

✅على الرغم انه عندما يكون MAP اقل من 82 يكون فرصه الوفاه عاليه في مريض التليف مازال clinical practice تقول بأنه يتم ايقاف دواء propranolol اذا كان  blood pressure اقل من 90/60 الذي تساوي MAP اقل من 70

طبعا ذا الموضوع في malpractice كثير

✅اولا راح اقتصر بالحديث عن اشهر نوع وهو ال
Type A lactic acidosis
الذي غالبا بيكون بسبب impaired tissue oxygenation نتيجه shock او sepsis

✅لطالما قلنا بأنه يجيب ان نعرف مخاطر الدواء قبل لأي غرض نستخدمه؟
✅اولا عشان تكون المعالجة ب sodium bicarbonate ناجحه ضروري اعالج السبب وحنا عرفنا فوق اهم سببين ل lactic acidosis او يمكن العلاج نفسه يفاقم  الحاله يعني يزيد من lactic acidosis كيف؟
✅عشان نجاوب نعرف كيفيه آلية عمل دواء Sodium bicarbonate  في خطوتين

✅First, it combines with a hydrogen ion to form carbonic acid (H2CO3)
✅H2CO3 dehydrates to carbon dioxide (CO2), and water (H2O). 

✅طيب CO2 لازم يطرد من خلال body  circulation ( يعني ضروري اصحح tissue hypoperfusion  واتأكد ان المريض adequate perfusion and ventilation لانه الهدف من المعالجة ب sodium bicarbonate المحافظه على arterial pH above 7.1 حتى نصلح السبب

✅طيب حتى لو كان في adequate perfusion  ممكن يحصل ارتفاع لحظي ل PCO2 لو أعطينا  infusion ب rat سريع   ⬅️لانه  CO2 readily penetrates cell membranes ⬅️مما يحصل worsen intracellular acidosis حتى لو ارتفع atrial blood pH

🛑طيب متى نعطي IV sodium bicarbonate?
✅هناك اختلاف متى بضبط ولكن اجمع اغلب الخبراء بأنه نعطيه في حالتين
✅severe lactic acidosis
ويعرف بانة يكون  PH اقل من 7.1 و serum bicarbonate  6 او اقل
✅less severe acidosis
لو كان PH مابين 7.1 إلى 7.2 مع وجود فشل كلوي حاد AKI

🛑طيب كم الجرعه ممكن في حاله sever acidosis نعطي
intravenous sodium bicarbonate bolus of 1 to 2 mEq/kg body weigh
✅اشهر تركيز موجود 8.4% طبعا بحجم 50ml؛يعني كل 1cc من تركيز 8.4% فيه 1mEq من sodium bicarbonate


✅The serum electrolytes and blood pH should be measured 30 to 60 minutes later, and the dose of sodium bicarbonate can be repeated if severe lactic acidosis (pH less than 7.1) persists.

🛑ماهي المشاكل التي قد تواجهنا أثناء المعالجة بIV sodium bicarbonate ??

✅اولا اذا كان PH  يساوي او اقل من 7.1 وكان bicarbonate level is greater than 6 mEq/L فهذا يشير إلى PCo2 ممكن يكون فوق 20 فهذا يدل انه المريض  inadequate ventilation وقد يكون لديه mixed metabolic and respiratory acidosis وإعطاء المريض infusion ب rapid rate ممكن يفاقم ال respiratory acidosis والحل انه ممكن mechanical  ventilation عشان إنقاص PCo2 ورفع PH

✅ثانيا مشاكل تتعلق ب rapid infusion bicarbonate
✅Effects on calcium, sodium, and extracellular fluid volume 

اي  ارتفاع في PH قد يصاحبه انخفاض في ionized calcium concentration و ارتفاع في serum sodium لان ال sodium bicarbonate بتركيز 8.4% يعتبر hypertonic solution

✅اي انخفاض في ionized calcium قد يصاحبه hemodynamic instability  لذا يجب قياس ionized calcium ومعالجته في حال كان هناك نقص ؛في حاله لم يتوفر ionized calcium  test  او تأخر في أخذه ممكن بأن انخفاض ضغط الدم او عدم تحسنه يعطيني مؤشر بانخفاض  ionized calcium وقد اخذ بعين الاعتبار اعطي المريض empiric calcium infusion

✅طيب في حاله إعطاء عده جرع من sodium bicarbonate  و PH لم يرتفع  فوق 7.1 وعالجات السبب الرئيسي ل lactic acidosis هنا ممكن اعطي المريض  continues infusions ولكن راح اخلي المحلول isotonic بدل ماكان hypertonic يعني راح أحضر 3 امبول من تركيز 8.4%  في 1 لتر من 5% dextrose 
في حاله حصول volume overload  او تدهورت وظائف الكلى اكثر هنا ممكن الجاء إلى الغسيل الكلوي

#Noncardiogenic_pulmonary_edema

✅ is identified clinically by the presence of radiographic evidence of alveolar fluid accumulation without hemodynamic evidence to suggest a cardiogenic etiology (ie, pulmonary artery wedge pressure ≤18 mmHg


✅The most common cause of noncardiogenic pulmonary edema is acute respiratory distress syndrome


✅Other less common causes include pulmonary edema due to pulmonary embolism and eclampsia, viral infections, pulmonary veno-occlusive disease, and transfusion-related acute lung injury.

✅بنسبه للتغذية الوريديه TPn التي تحتوي على glucose ممكن  تعمل hyperglycemia  في الشخص الغير مصاب بالسكري اذا أعطيت  ب rate أعلى من  4 إلى 5mg/kg/per minute  لان هذا rate يتغلب على قدره الجسم في استقلاب الجلوكوز حتى لو اعطي معه اعلى جرعه من الأنسولين
✅فمثلا الشخص الذي يزن 70 كيلو اذا اعطي TPN الذي تحتوي على dextrose 5% ب rate 5mg/kg/per minute الذي قد تساوي  5.6_ 7ml/minute    بمجموع 336 إلى 420ml في الساعه  الواحده قد يعمل hyperglycemia
✅لذا ال safety rate من TPN التي تحتوي على glucose  ممكن يكون 2.5mg/kg/per minute

🛑بنسبه ل maintenance fluid التي نعطيها لمريض المستشفى الغير قادر على تناول الاكل نعطيه في الغالب لترين في اليوم من Norml saline مع dextrose 5%  مع اضافهmEq ٢٠ من kCl في كل لتر

✅في حاله كان هناك ارتفاع في درجة حراره جسم المريض قد نزيد  من 100 إلى 150 مل في اليوم لكل درجه حراره فوق 37c

اذكى رجل خاض خداع استراتيجي مع العدو هو يحي ال سنوار حفظه الله شاهدت مقابلة له وهو في سجون الاحتلال وكان يقول بأنه يتمنى هدنة طويله بين إسرائيل والقطاع وانه لن تستطيع حم اس تدمير الاحتلال وهذا جعل جهاز الموساد يطمئن عند تولي الس نوار كقائد سياسي ل حم اس عام ٢٠١٧ والرجل كان يخطط من ذو خروجه ل معركة طو فإن الاقصى ،حفظه الله ونصره ❤
