J wave from J point.
ST/T ratio (more T wave height and lower ratio (<0.25) seen in early repolarization), a helpful ECG cue to differentiate it from acute pericarditis.

❣️ECG Findings in Digoxin Toxicity⬇️

➡️There is no specific arrhythmia for digoxin toxicity rather a range of arrhythmias can be present such as various degrees of AV block, premature ventricular contractions, bradycardia, and even ventricular tachycardia. Cardiac arrhythmias are the main cause of death for those with digoxin toxicity.

➡️Certain ECG changes with digoxin may purely indicate a therapeutic effect (such as the ST depression and T wave flattening), while other changes (such as T wave inversion and arrhythmias) indicate toxicity.
🫀ST depression - down-sloping or scooped
🫀T wave changes - flattening, inversion or biphasic T waves

➡️Mechanisms of Cardiac Toxicity:
1. Due to Increased Automaticity
🫀QT shortening (due to reduced ventricular repolarisation time)
🫀Premature beats - atrial, junctional or ventricular
🫀Tachyarrhythmias - atrial tachycardia, accelerated junctional tachycardia

2.Due to Reduced Conduction
🫀Prolonged PR interval
🫀Bradyarrhythmias - sinus bradycardia, bundle branch block, AV block

➡️DIGOXIN-INDUCED ARRHYTHMIAS
🫀Sinus bradycardia, tachycardia, block, and arrest
🫀Ectopic atrial tachycardia with block
🫀Atrial fibrillation and flutter
🫀Atrioventricular nodal block
🫀Junctional rhythm, tachycardia, and bradycardia
🫀Ventricular arrhythmias
• Premature ventricular complexes
• Ventricular tachycardia
• Ventricular fibrillation
• Bidirectional ventricular tachycardia
NB:These ECG changes can arise with digoxin within normal therapeutic levels.
